Boston: Age Strong Blog: September is National PACE Month

~ As National PACE (Program of All-Inclusive Care for the Elderly) Month kicks off in September, Uphams Community Care PACE held a special celebration on September 17 to honor the occasion. The event was attended by Age Strong Commissioner Emily Shea, who took the opportunity to meet with PACE participants and connect with program staff.

PACE is a unique healthcare model that aims to support older adults in maintaining their independence by allowing them to stay in their own homes and communities instead of being placed in nursing homes. Upham's Community Care PACE, an all-inclusive medical care program for individuals aged 55 and above, offers a range of services including primary and preventative care, as well as community-based acute and long-term care. This program serves a diverse population, all of whom are on Medicare and Medicaid.

Located in Dorchester, Uphams Community Care PACE boasts an adult day health center and medical facilities. Other locations include Nubian Square in Roxbury and Jackson Square in Jamaica Plain.

To qualify for PACE, individuals must be 55 or older, reside within the PACE service area, and meet clinical criteria for nursing facility care.
